Only beneficiaries and trustees of Aurelia Mitchell Trust can request access for the court-ordered confidential information using the Beneficiary Request Access Form located at https://aureliamitchelltrust.com/registration/.
In order for the trustees of Aurelia Mitchell Trust to begin the validation process, you consent to allow Aurelia Mitchell Trust to request your Personally Identifiable Information (PII) including your full legal name (first, middle, and last names), email address, phone number, and last four of your social security number. All submissions are sent using SSL technology and are stored securely using encryption on our JotForm account, which will also record your IP address. The Beneficiary Request Access Form and any other of our public JotForm forms are securely encrypted on our JotForm account. Once trustees of Aurelia Mitchell Trust validate your Personally Identifiable Information (PII), the trustees will create your website account, then delete or remove your Personally Identifiable Information (PII) from our JotForm account.
Website logs may record your IP address and other meta data information.
Your Personally Identifiable Information (PII) is not shared or sold to third parties. We operate the Aurelia Mitchell Trust website.

Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
