Affidavit of Heirship for New Beneficiaries
Documents required to claim heirship include complete and notarized affidavit of heirship; a copy of the death certificate; and copy of birth certificate, valid picture ID, and social security card for each child of the deceased heir. If a mailing address differs from the one on the picture ID, the beneficiary must write the current address, and sign and date the change on the sheet with the copy of picture ID.
Note: Affiants must be disinterested (persons who can never benefit from the trust). Affiants’ information is required for question #1. The affiants’ signatures must be notarized.
