Registration for Beneficiary Access

In order to process your request for Beneficiary Access, please review the Privacy Policy, then complete the following required information:

1. Full Legal Name
2. Last 4 of Social
3. Email Address
4. Phone Number

Once your request has been submitted, the Trustees will respond as soon as practical, usually within ten (10) business days. Incomplete and non-qualifying submissions will not be processed.

If your request for Beneficiary Access is approved, the Trustees will provide your account information for Beneficiary Website Access.
